Welcome Packet

We are excited that you have decided to join the Oxbow community!

In order to participate in Oxbow's programs, each family is required to complete the Welcome Packet. These completed forms can be returned to The Oxbow School's office or via email to admissions@oxbowschool.org. Please ensure you have reviewed, signed, dated and submitted all the forms on the specific checklist in (July 26th for Fall 2022 and December 1st for Spring 2023).

It is imperative that both parents and students review the information in the packets. We prefer you email or mail all the completed forms together at one time. Please return these forms in advance of the deadline so that Oxbow can prepare to welcome the students by our program's start date. We understand things are very much up in the air for many of you and that you may need time to see how things develop and make plans for your school year.

The Welcome Packet includes the following forms, please use the appropriate links for Fall and Spring below this section:

  • Course Selection Form (for Fall and Spring semester students)
  • Dormitory Questionnaire
  • Health Forms:
    • Medical History and Wellness Information Forms
    • Health and Permission Statement
    • Annual Physical Exam
    • Student Immunization Form (must be completed by your physician within 12 months of the program start date)
    • Copy of Medical ID Card (Student insurance card, front and back)
    • Copy of COVID-19 Vaccine Card (with all booster information)
    • COVID-19 Testing Consent Form
  • Family Contacts and Off Campus Permissions Form
  • Travel Survey
